Interested Consulting firms must provide the following information as a part of the EOI:
1) Organization Brochures describing its facilities and area of expertise.
2) General experience of Minimum 05 (Five) years (Years Counting backward from the date of publication of EOI.)
3) Specific experience in any government Research/Survey activities with successful completion of at least 01 (One) single contract valued not less than -Tk. 1.50 Cr. (One crore fifty lac) within last 03 (Three) years as a Research firm. (Years Counting backward from the date of publication of EOI.)
4) physical facilities (office, equipment, A.V. materials and transport).
5) Trade license valid up to 2023.
6) Income tax payment certificate for TY 2021-22.
7) VAT registration certificate.
8) Credit facility certificate from a schedule bank of Tk. . 1.50 Cr. (One crore fifty lac) (Any credit facility certificate issued before 28 days prior to last date of submission of EOI will not be accepted).
9) Name & CV of 05 (five) professional staff & central key personnel describing their qualifications along with his/her last academic certificate, photo, contact number and email address who will be involved in the assignment.
10) List of available manpower, along with their Photograph, describing their education qualification, designation in the organization.

1. Documents should duly sealed and initialed by the owner or his/her authorized representative.
2. EOI can only be submitted at the office of the Line Director, Lifestyle and Health Education & Promotion, Bureau of Health Education, Directorate General of Health Services. Mohakhali, Dhaka.
3. The Consulting firms should prepare and submit 03 (Three) copies (01 original & 02 Photocopy) of EOI in response to this advertisement. Any Class One Gazetted Official must attest all the photocopy of the legal documents and certificates.
4. During the financial year 2021-2022 a firm cannot submit EOI for more than 01 (one) package.
