To perform study on the epidemiological situation of major NCDs including mental health among the people of hard –to-reach area and to examine their life –style and social backgrounds which may affect their health status as well as identify various risk factors which needs to be taken into account to develop effective control measures for community level comprehensive intervention through Community Health Workers (CHW) at community level . |

The applicant shall have to submit the EOI with the following documents; (1) Category of the firm; Research/ Survey organization/ Consulting firm; (2) Bried description of the firm with proof of working in the relvant areas and of conducting research /Survey; (3) Valid & Update Trade license / Registration; (4)Updated income Tax certificate/ Certificate of VAT; (5) 8 years’ experience in the relevant Survey/ Rsearch; (6) List of major clients working with in last 8 years contract reference; (7) Bank solvency certificate; (8) Nome of the central kdy personnel with C.V who wil be involved with the assignment; (9) Relevant document in favor of organization strength of the firm |
