To establish a people-centered care system with following activities: (1) To implement a technology-based screening process of NCDs and its risk factors using smart device and app at the community level as well as facility, (2) To create digital health account for each adult of the selected area and to register each patient diagnosed as hypertension and diabetes, (3) to establish a screening- diagnosis-management- frferral-fllow up chain following national protocol using IoT enabled digital health technology the existing primary health care infrastructure. |
